Assistant Practitioner - Sidmouth
We have an exciting opening for an enthusiastic and motivated Band 4 Assistant Practitioner to join an integrated rehabilitation pathway across community services. This is a fantastic opportunity to join a dynamic and forward thinking service with high morale and a commitment to supporting people at home. You will be well supported and encouraged to continuously develop to meet the needs of yourself and the service.
The Seaton, Axminster and Sidmouth Cluster has an attractive mix of rural villages and coastal towns whilst being only 30mins drive from Exeter. The Cluster is set in a landscape of outstanding natural beauty, and part of the Jurassic Coast World Heritage Site. The largest landslip in the UK provides the landscape for coastal walks, fishing harbours and beautiful beaches. Inland, the market town of Axminster provides excellent rail links to London and Exeter.
You will be part of a multi-disciplinary, integrated team and will have the chance of working with patients in their own homes and care or nursing homes, who present with a wide variety of long term conditions including complex neurological and falls. The emphasis is to maximise their independence and quality of life through assessment and goal based rehabilitation to prevent hospital admissions and support the discharge process from hospital. Clinical supervision will be provided by a qualified Occupational Therapist, Physiotherapist or Rehabilitation Nurse.

Community Nurse Specialist - Sidmouth
We are seeking a Band 6 Community nurse specialist to lead our small Sidmouth Community Nurse team.
We are affiliated with the Sid Valley Medical Practice and visit those patients that cannot get out of their houses to provide nursing interventions.
